Please include:
- Your name and date of birth
- Your account number (found on your billing statements in the Tilt app)
- The name of the credit reporting agency with incorrect information
- The specific information you believe is incorrect, with enough detail for us to locate the item(s) you're disputing (date, description, etc)
- A copy of your credit report with the disputed information highlighted (optional)
- What you believe is inaccurate or incomplete about our reported information, and why
